Auto Self Service Station

We are pleased to introduce our brand-new Auto Self-Service Station, designed to make your visit smoother and more efficient. The station is clearly labelled and easy to use, allowing you to complete common tasks without waiting at reception. Here’s what you can now access without waiting in the queue:

• Sample Drop-Off

  • • You can now drop off your samples easily using the self-service station.

    • Please complete a sample tracking ticket before placing your sample in the box. This form is essential for the surgery to keep accurate records of all samples dropped off. In the rare event a sample goes missing during transport to the lab, the tracking information allows us to investigate and communicate with you appropriately.

    • Important: Samples must be dropped off before 5pm. Any samples left after this time may compromise test results.

• Sample Bottle Collection

  • • Need a sample bottle? Help yourself directly from the self-service wall — no need to wait at reception.

• Document Pick up and Drop Off 

  • • The following documents can now be picked up anytime from the self-service station:

      • New patient registration packs

      • Third party consent forms

      • Proxy individual access forms

      • Care proxy access forms

      • Repeat Prescription drop off

Once completed, please place all forms in the ‘Documents & Prescriptions’ letter box located at the self-service station.

Kind reminder:

• Please help us keep the area clean and tidy for everyone. Do not leave cups, rubbish, or personal items on or around the cubes.
• Please do NOT drop off samples using the ‘Document & Prescription’ letter box. Sample drop off box is separate to avoid contamination.
