Contact Reception

If you are contacting the surgery for the following, please refer to the relevant areas of the website.

  • GP Appointment Request – Please submit these via Rapid Health
  • Nurse / HCA appointments – Requests for these appointments must be made via Manage Your Condition
  • Follow Up Appointment – Requests for these appointments must be made via Manage Your Condition
  • Tests Results – Please access your results through the NHS App or Patient Knows Best.
  • Fit note / sick note – Requests for fit notes must be submitted via Rapid Health
  • Referral Query – Please note that the surgery does not chase referrals on your behalf. Once your referral has been sent via eRS, you will receive the relevant paperwork containing all the information required when contacting the referral provider for updates. They will request the UBRN number, please keep this at hand when contacting the relevant department for referral query.
  • Administrative Requests (Forms /Letters) – These requests should be submitted via the Non-NHS Services section on the surgery website.
  • Access to Medical Records – To access your medical records, or those of someone you care for, please complete the proxy access forms available on the surgery website under ‘NHS App Access
