NHS Patient Transport Services (PTS) Request
NHS Patient Transport Services (PTS) provide transport for patients who have a medical or mobility-related need that means they cannot reasonably travel to their healthcare appointment by other means.
Patient Transport is not normally available simply because you do not have access to a car, public transport, or someone who can drive you.
Who may be eligible?
You may be eligible for NHS Patient Transport if, because of your medical condition or disability, you:
- Are unable to travel safely using ordinary public or private transport.
- Need specialist support or assistance during your journey.
- Need to travel lying down or require medical equipment or monitoring during transport.
- Have significant mobility difficulties that prevent you from travelling independently.
- Have a condition that makes travelling by normal transport unsafe or unsuitable.
- Need an escort because of your medical or communication needs.
Eligibility is assessed based on your individual medical and mobility needs.
Please request transport in advance
Transport must be requested at least 1 week before your scheduled appointment.
Please complete the form below as soon as you receive your appointment date to allow enough time for your transport to be assessed and arranged.
Important: We cannot book next-day transport
We are unable to arrange Patient Transport for appointments taking place the following day.
For example, if your appointment is on Tuesday, you must request your transport no later than the previous Friday, subject to the service’s operating hours and any applicable booking requirements.
