Important Information for New Patient

Registration Process:

As it often takes time for your medical records to be transferred from your previous practice, we kindly ask all new patients to complete a health questionnaire. This helps us understand your medical history and current health needs while we await your records.

Total Triage System:

Our practice operates a Total Triage (TT) system. This means all appointment requests must go through a screening process by a clinician to ensure you receive the most appropriate care. You can request an appointment via:

  • PATCHS: An easy-to-use online service where you can submit your symptoms or queries. You can access PATCHS through our practice website, available from 7am to 6pm (Monday – Friday). If you are having trouble using PATCHs, please see our IT support page.
  • Telephone: If you are not comfortable using technology, or if you are unable to access PATCHS, our friendly reception staff are happy to assist you in booking an appointment over the phone. Please call us during our operating hours (Monday to Friday, 8am – 6pm)

Once your request is submitted, we will assess your needs and arrange an appointment with the appropriate healthcare professional or provide advice on managing your condition.

Our practice is proud to offer a wide range of healthcare services, including:

  • Phlebotomy Services: Blood tests for adults and children aged 5 and above.
  • ECG: Heart monitoring tests to assess and diagnose heart conditions.
  • Health Checks: Routine health assessments to monitor your general well-being.
  • Chronic Condition Management: Ongoing care and support for conditions such as asthma, diabetes, heart disease, and more.
  • GLD/Insulin Initiation: Guidance and support for initiating GLD (Glucose-Lowering Drugs) and insulin therapy for diabetic patients.
  • Physiotherapy: Assessment and treatment for musculoskeletal issues, helping to improve mobility and reduce pain.

A Training Practice:

We are proud to be a training practice, meaning we play a key role in developing the future of healthcare. You may occasionally be seen by GP trainees or medical students under the supervision of our senior doctors. We assure you that they are fully qualified to help with your care, and you will always be supported by experienced healthcare professionals.

Named GP

Named GP

As part of the commitment to more personalised care for patients, NHS Employers and the General Practitioners Committee of the British Medical Association have agreed that all patients will have a named accountable GP.

The Named GP will:

  • Take lead responsibility for ensuring that all appropriate services required under the contract with the practice are delivered to you
  • Where required, based on the professional judgement of the ‘named’ GP, work with relevant associated health and social care professionals to deliver a multidisciplinary care package that meets your needs.
  • Ensure that your physical and psychological needs are recognised and responded to by the relevant clinicians in the practice
  • Ensure that patients over 75 years of age have access to a health check if requested, which is already a requirement of the GP contract regulations. ​​​

St Martin’s Medical Centre will ensure that there is a named accountable GP assigned to each patient.

New patients will be allocated a GP at the time of registration.

Your named accountable GP will be the same as your usual GP however this does not affect your ability to see any GP of your choice as you currently do.
