Ordering Repeat Prescriptions
If the pharmacist has prescribed regular medication then you can order a repeat prescription. If you are a new patient you will need to speak to the doctor first, so that your prescription can be added to our records.
Order Repeat Prescription
You can order your repeat medication through the following methods:
- Complete the right hand side of your previous prescription.
- Write out your request on the correct form available at reception and hand it over to the reception staff. You may also drop your repeat request in the letter box just outside the surgery doors
Request for repeat prescriptions will not be accepted over the phone to avoid any errors.
Test Results
We will contact you by telephone, or by letter, only if a result is abnormal and you require treatment or further investigations.
You will not be contacted if your result is normal.
Accessing Results Online
Once a doctor has viewed your test results, you may access them:
- Through your NHS account, on the NHS app or website
- Through your Patients Know Best account
- Through your Patient Access account
Please be aware that to view these records online, you must have an account created and verified for each application you wish to use. If you are having issues with account verification, please contact the surgery (01895 632410) and ask for reception to inform the IT support team to assist you.
Discuss Results
If you would like to discuss your test results with a GP, please complete an online consultation using PATCHs. A member of our staff will then contact you to discuss your results.
Samples
Pathology Samples
If a doctor or nurse has asked you to provide a pathology sample (e.g., urine, stool, sputum), please hand it to reception before 5:00 pm, Monday to Friday.
We kindly request that pathology samples are not posted or left in the letter box to avoid contamination.
Sick (Fit) Notes
Sickness for Seven Days or Less
If you’re off sick for seven days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

Sickness of More than Seven Days
If you are sick and off work for more than seven days, your employer will normally ask you to provide a medical certificate from your GP. When you need a certificate will also depend on your employer’s company policy on sick leave (or sickness absence). This policy should tell you how many days you can be off sick before you need a note.
To find out about your employer’s policy :
- Ask your team leader or supervisor, or
- Speak to someone from your human resources (HR) or personal department
When your Medical Certificate Runs Out
If your certificate runs out but you are still sick, you will need to consult the doctor again before you can get a further sick note. Renewal of an existing medical certificates cannot be completed until the current certificate runs out.
Medical certificates can be back-dated so it is not necessary to make an emergency appointment to renew your certificate. If you attend an emergency appointment for the purpose of getting a medical certificate, you will be asked to re-book.
FIT notes for Children
Please note that St Martins Medical Centre does not provide examinations and sickness certificates for school children.

- It should be noted that GPs do not provide sick notes for school children
- When children are absent from school owning to illness, schools may request a letter from a parent or guardian, and this is no different during an exam period
- However, children who have missed exams due to illness are frequently told by schools that a note from a doctor is required; but this cannot be provided by a GP
- Aside from the fact that parents/guardians are responsible for excusing their children from school, GPs cannot provide retrospective sickness certification
- When a child suffers from a long-term condition, any certification will be provided by the responsible specialist
- GPC has sought and received confirmation from the Office of the Qualifications and Examinations Regulator that Awarding Organisations make no requirement for pupils to obtain a medical certificate in support of their application, but this may take the form of a statement by the school
- The Joint Council for Qualifications has confirmed that as far as they are concerned, if a student was absent from an examination as a result of illness and has the support of the school or center to be absent, special consideration will be granted on that basis
- Awarding Organisation do not insist that medical proof is provided

Please note that insurance reports incur a fee; contact reception to confirm the charges. All requests require a valid request and consent form and are processed within 28 working days.
